Evidence-Based Practice Proposal - Section E: Implementation Plan

Research Proposal Instructions:

In 1,000-1,500 words, provide a description of the methods to be used to implement the proposed solution. Include the following:
Describe the setting and access to potential subjects. If there is a need for a consent or approval form, then one must be created. Although you will not be submitting the consent or approval forms in Topic 5 with the narrative, you will include the consent or approval forms in the appendices for the final paper.
Describe the amount of time needed to complete this project. Create a timeline. Make sure the timeline is general enough that it can be implemented at any date. Although you will not be submitting the timeline in Topic 5 with the narrative, you will include the timeline in the appendices for the final paper.
Describe the resources (human, fiscal, and other) or changes needed in the implementation of the solution. Consider the clinical tools or process changes that would need to take place. Provide a resource list. Although you will not be submitting the resource list in Topic 5 with the narrative, you will include the resource list in the appendices for the final paper.
Describe the methods and instruments, such as a questionnaire, scale, or test to be used for monitoring the implementation of the proposed solution. Develop the instruments. Although you will not be submitting the individual instruments in Topic 5 with the narrative, you will include the instruments in the appendices for the final paper.
Explain the process for delivering the (intervention) solution and indicate if any training will be needed.
Provide an outline of the data collection plan. Describe how data management will be maintained and by whom. Furthermore, provide an explanation of how the data analysis and interpretation process will be conducted. Develop the data collection tools that will be needed. Although you will not be submitting the data collection tools in Topic 5 with the narrative, you will include the data collection tools in the appendices for the final paper.
Describe the strategies to deal with the management of any barriers, facilitators, and challenges.
Establish the feasibility of the implementation plan. Address the costs for personnel, consumable supplies, equipment (if not provided by the institute), computer-related costs (librarian consultation, database access, etc.), and other costs (travel, presentation development). Make sure to provide a brief rationale for each. Develop a budget plan. Although you will not be submitting the budget plan in Topic 5 with the narrative, you will include the budget plan in the appendices for the final paper.
Describe the plans to maintain, extend, revise, and discontinue a proposed solution after implementation.
From nine questions I need only 3,4,5,6,9 attached below; Ruberic also attached.
Answer: Tools: Transtheoretical model is the theoretical frame work used and pre and post test assessment questionnaire is used
4. Describe the methods and instruments, such as a questionnaire, scale, or test to be used for monitoring the implementation of the proposed solution. Develop the instruments. Although you will not be submitting the individual instruments in Topic 5 with the narrative, you will include the instruments in the appendices for the final paper.
5. Explain the process for delivering the (intervention) solution and indicate if any training will be needed.
Answer: Training may needed for time management and foot care assessment to nurses through online
Answer: Use control groups with one time education and experimental group has every week 60mint educational group sessions for six months
9. Describe the plans to maintain, extend, revise, and discontinue a proposed solution after implementation.
You are required to cite five to 10 sources to complete this assignment. Sources must be published within the last 5 years and appropriate for the assignment criteria and nursing content.
Prepare this assignment according to the guidelines found in the APA Style Guide, located in the Student Success Center. An abstract is not required.

In diabetic adult patients from 18-65 years of age who are at the risk of diabetic foot ulcers(P) what is the effect of ongoing diabetic and foot care education (I) on reducing the risk of developing Diabetic Foot ulcers(DFU’S) (O) compared with diabetic patients who have one time education session on this topic (C) over a 6-month (T) time frame.
The population of focus is the diabetic patients who are at the age of 18-65 years, who had diabetic foot ulcers or at risk of getting diabetic foot ulcers.

1 In order to conduct the research experiment under this proposal, a transtheoretical model would be utilized for the framework, while pre and post-assessment questionnaires would be utilized as a measurement too. The transtheoretical model aims to assess how people are capable and willing of changing their behaviors are relative to a particular trait that is sought. Accordingly, this model has five states, namely (1) precontemplation, (2) contemplation, (3) preparation, (4) action, and (5) maintenance CITATION Lar16 \l 1033 (Laranjo, 2016). These stages differ based on the ‘readiness’ or ‘willingness’ of the individual to change, and will thus be the subject of measurement for the research.
2 In order to properly assess the resulting change in behavior on the part of the patients, the researcher would use two measurement tools (1) surveys and (2) patient health reports. On the one hand, the survey questionnaires (pre and post-assessment) used to contain both quantitative and qualitative questions that seek to measure the patient’s motivation CITATION Edi15 \l 1033 (Edinger, et al., 2015). Although the questions are mostly in the form of ‘multiple choice’ questions measuring quantitative data, additional the questions which can be gathered through interview would be added. On the other hand, the patient health report would also determine the number of patients who have developed DFUs after the 6-month period.
3 In order to deliver the intervention, additional online training for the nurses would be needed in order to teach them two things (1) time management and (2) instruction methods and procedures for the different times periods. On the one hand, the training for time management is required of both groups of nurses catering for the different study groups CITATION Hol13 \l 1033 (McCutcheon, Lohan, Traynor, & Martin, 2015). On the other hand, each group of nurses would be taught how to instruct the two different sets of ongoing diabetic and foot care education for each of the groups.
4 In order to collect the data, the population of respondents would be grouped into two, namely (1) the ‘...
