According to the HMO’s pharmacy process of placing prescriptions. There a number of challenges that come up (Daniels, 2015). The flawed process basically involves going to the doctor with an issue, the doctor writes a shorthand prescription note for them to take to the pharmacy, the patient hands the note to the pharmacist, the clerk misreads the note and stamps, clerk then gives the wrong description to the pharmacist, the pharmacist then reads and dispenses the prescription, then hands the wrong medication to the clerk who then gives the same to the patient, the patient then gets sick from using the wrong medication (Daniels, 2015).
