When pediatric clients present with mood disorders, the process of assessing, diagnosing, and treating them can be quite complex. Children not only present with different signs and symptoms than adult clients with the same disorders, but they also metabolize medications much differently. As a result, psychiatric mental health nurse practitioners must exercise caution when prescribing psychotropic medications to these clients. For this Assignment, as you examine the client case study in this week’s Learning Resources, consider how you might assess and treat pediatric clients presenting with mood disorders.

Note: This Assignment is the first of 10 assignments that are based on interactive client case studies. The process of diagnosing and treating clients with mood disorders is sometimes very complex. Treatment of such clients depends on particular illnesses and symptoms that are present. A therapy session should be conducted first before making any decision about the treatment process. This paper will comprise of an assessment of a client who is an African American suffering from depression.
Background of the client
The symptoms exhibited by the eight-year-old African American male child can be associated with depression. The client is complaining feeling sad and a report from the teachers shows that the child is withdrawn from his peers. Most children when they are depressed it becomes very difficult for them to fit in the company of their peers. Besides, his mother has reported a decreased level of appetite and some occasional irritation which are evident.
Mental status exam
The child does not exhibit any signs of contemplating suicide although he admits of thinking about his death. There is also an absence of delusions and paranoia as well as hallucinations. The depression rating scale shows a score of 30 and the mood is sad (Poznanski & Mokros, 1996).
Decision 1: Begin Zoloft 25 mg orally
The dosage is moderate for children between 7-15 years of age bracket. Zoloft is the most popular anti-depressant used. The reason for recommending a dosage of 25mg of Zoloft is because medicine is known to treat major depressive disorders such as post-traumatic disorders and social anxiety disorders. Zoloft raises the level of serotonin in the human brain. Zoloft also are safe for use for children between 6-17 years. Thus, the medicine will show significance results to the client.
By prescribing the medicine I expected the side effects to start reducing by the first week. I expected the victim to show some response and effectiveness. However, the result shows no response after the dosage and there were no changes detected. The reason for the difference in the effectiveness of Zoloft after four weeks is because the drug sometimes shows response after 8 weeks of continuous usage (Leo, 2006). In this case, the client had only used the drug for 4 weeks.
