Inter-agency working is when people from different agencies or organizations come together to join efforts, resources and decision making to solve a problem that focuses on improving the services or conditions of a certain group whereas person-centred care is caring for using health and social services to plan, develop and monitor their care to ensure that it meets their needs. In general, person-centred care does not only end in providing individuals with what they need, it involves considering their values, lifestyles, desires, social and family situations and involving them to develop appropriate solutions.
Falls in older population are becoming a common, serious and growing concern in public health. Majority of older adults above 65 years old fall and these falls have significant impacts such as fatal injuries, loss of independence and decreased morbidity. Only a few of seek medical care after falling. The aging population is increasing at a higher rate and it vulnerable to various problems such as balance, mobility and weak muscles which increase their vulnerability to falling. Moreover, majority of this population is living with chronic diseases such as cancer and diabetes whose treatments and medications are likely to increase the risk of falling. Therefore, the increasing rate of falls, long-term effects and costs is likely to have a significant impact on the healthcare system with time.
