This is an evidence-based research project. Please write accordingly. It is the week 4 assignment, a continuation from the previous weeks. It is a proposed research project in a hospital.
This is the PICOT formulation of 2nd week.
“In hospitalized elderly fall risk patients with fall prevention protocols in practice (P) how does hourly rounding (I) compared to no hourly rounding (C) affect the fall rates (O) within 6 months of initiation (T) “?.

“In hospitalized elderly fall risk patients with fall prevention protocols in practice (P) how does hourly rounding (I) compared to no hourly rounding (C) affect the fall rates (O) within 6 months of initiation (T) “?
Introduction
The populations with greater risk of suffering falls in hospitals are children under 7 years of age and with a greater incidence and severity the patient over 65 years of age, and especially those in long term care. The risk factors for falls are intrinsic, which is directly related to the patient or extrinsic, which is related to the environment. Rounding is one of the interventions that can decrease patient fall rates it is intentional and person-centered to support safe and effective practice.
Proposed Solution:
Purposeful rounding is proposed as the solution to reduce patient falls, and while there are nonfatal fall injuries, the injuries are fatal in surgical units and falls increase health care costs. The elderly are at high risk of patient falls and keeping track of patient falls is important to improve patient health, safety and the quality of care. Rounding ensures that the highest standards of care are prioritized, and this will also reduce the medical costs linked to patient falls (Jackson, 2016). The case for monitoring rounding is that the patient outcomes improve, and the patients spend less time in the hospital and there is intervention to reduce the risk of patients health worsening. There is patient prevention falls protocols and prevention bundles that integrate multiple interventions including rounding and this will be considered when integrating the intervention.
Organization Culture
The organization emphasizes nurse-led initiatives to improve health care outcomes and quality of care and rounding is an evidence-based strategy that achieves this. Nurses are required to check on patients at specific times and take actions to improve the quality of care and patient safety. Preventing patient fall is a quality measure and there will be patient education to improve health outcomes and there is commitment to safety and quality (Mitchell et al., 2014). When the changes are implemented there will be reduction in the fall-related outcomes. There are safety and risk management concerns and patient fall prevention is prioritized so that there are lower patient fall rates. The hospital fall prevention strategy is consistent with the aim of implementing appropriate interventions to improve the quality of care.
