Nurse informaticists and healthcare leaders formulate clinical system strategies. As these strategies are often based on technology trends, informaticists and others have then benefited from consulting existing research to inform their thinking.

In this Assignment, you will review existing research focused on the application of clinical systems. After reviewing, you will summarize your findings.

To Prepare:

  • Review the Resources and reflect on the impact of clinical systems on outcomes and efficiencies within the context of nursing practice and healthcare delivery.
  • Conduct a search for recent (within the last 5 years) research focused on the application of clinical systems. The research should provide evidence to support the use of one type of clinical system to improve outcomes and/or efficiencies, such as “the use of personal health records or portals to support patients newly diagnosed with diabetes.”

1 In a few sentences, explain the appropriate drug therapy for a patient who presents with MDD and a history of alcohol abuse. Which drugs are contraindicated, if any, and why? Be specific. What is the timeframe that the patient should see the resolution of symptoms?
The best pharmacological therapy for a patient with both MDD and a history of alcohol abuse should address the palliative symptoms and support the patient's non-pharmacological treatment if any. Accordingly, since the patient is diagnosed with major depressive disorder, a good combination of both pharmacological and non-pharmacological treatment (i.e., cognitive behavioral therapy) should be used. Nonetheless, the following are the appropriate drug therapy for such patients. 
           This, first FDA-approved medication for patients with both MDD and alcohol abuse is Naltrexone, which may either be oral or injectable. It is considered a long-lasting opioid antagonist, which helps reduce the feeling of "pleasure" received by patients who are suffering from alcohol abuse (Blum et al., 2018). 
           Another drug therapy that might be used is a combination of acamprosate and escitalopram. While acamprosate does not reduce the risks and dangers of alcohol withdrawal, this medication is used to reduce alcohol use disorder regardless of whether there is a presenting symptom of depression (McHugh & Weiss, 2019). Accordingly, the combination of escitalopram would help treat the depressive symptoms. 
           Finally, the patient can also be subjected to the use of disulfiram (aka Tetraethyl thiuram disulfide or Antabuse).
