Background question: What is the effect of proper patient education among the elderly patients with Congestive Heart Failure on mortality, readmission and healthcare cost? PICOT Question: In patients (over 65 years) diagnosed with CHF and hospitalized (P), will a discharge plan aimed at educating the patients and relatives on medication adherence, diet and management of CHF (I) be more effective in improving patient outcomes mortality and reducing need for re-hospitalization and health care costs (O) than a discharge plan without these elements (C) for a period of 1 year post-discharge (T)? please help me to Identify a "theory" that can be used to support my proposed solution in the PICOT Question. that is: Will a discharge plan aimed at educating the patients and relatives on medication adherence, diet and management of CHF, be more effective in improving patient outcomes mortality and reducing need for re-hospitalization and health care costs .

Orem’s Self-Care Theory
The identified theory is Orem’s Self-Care Theory that was developed by Dorothea Orem. The major assumptions of this theory are that (i) people have to be responsible, independent and self-reliant for their own care and others in their family who need care; and (ii) nursing is a type of action since it involves interaction between at least 2 people (George, 2011). (iii) An individual’s knowledge and understanding of potential health problems is essential in order to promote self-care behaviors. (iv) The other assumption is that successfully meeting development as well as universal self-care requisites is a vital element of primary care prevention and ill-health (Parker, 2010). Orem’s Self-Care theory basically interconnects concepts in such a manner as to create a dissimilar way of looking at a certain phenomenon (Nursing Theory, 2014). The rationale for selecting this theory is that it can be used to promote self-care by the patient to adhere to medication and get proper diet, and to encourage relatives of the patient to care for the patient.
