Introduction:

In the second step of the EBP project, we searched the research databases for studies that answered our PICOT question. We then evaluated the evidence obtained in those research studies and determined how it applied to our PICOT question.

In the third step of the EBP project, we will describe the proposed practice change. We will also discuss how to assess clinical expertise, patient preferences and values, use an EBP model to guide the development of plans to implement the proposed practice change, and discuss communication, evaluation, and sustainability plans.

Patient falls to the largest category of hospital incidents. It refers to a sudden descent of the patient to the ground, floor, or extension of the floor with or without injury. The hospital falls, and injury rates from the falls vary depending on patient risk factors, patients' population, and the presence of fall control factors (Morris & O'Riordan, 2017). A previous fall increases the chance of another fall happening. Therefore, there is a need to put in place interventions to prevent these falls. These falls affect both inpatients and outpatients in hospitals, and if left unattended, they might increase fatality rates through unprecedented accidents. Nurse practitioners have to develop interventions that will eliminate or minimize falls in hospitals.
The first step to developing the intervention is collecting evidence that will help prevent patient falls. These pieces of evidence will be gathered from clinical experts and other stakeholders (Callis, 2016). First, it is important to evaluate whether the clinical problem is worth a priority, as the subsequent paragraphs discuss. Once the problem has been marked as a priority, the next step is to gather evidence from stakeholders. The process will take six months to get the best shreds of evidence and exhaust all the possible strains of evidence to use. For clinical officers, it would give significant evidence if they are approached based on their designations. This means that all nurses across all the departments should be approached to give their report on patient falls. Some of the questions to ask clinical experts are;
* Have you ever witnessed a patient fall in this health facility?
* What is the frequency of patient falls?
* Patients in which wards often experience falls?
* Are falls frequent on patients suffering from certain conditions? If yes, name the conditions in order of the frequency of falls.
Patients are the primary victims of falls, and they are instrumental in the collection of evidence. The best way is to give patients from all the departments and wards a chance to respond to the questions to ensure that enough evidence is collected. Have a schedule of collecting the evidence from all the departments beginning with the children's wards, maternity wing, wards with patients of accidents, general wards, and outpatients. Among the questions to ask patience are;
* Have you ever fallen as a patient?
* Have you ever witnessed a patient fall in the ward? If yes, how often?
* How do nurses respond to incidences of falls?
* What do you think are the reasons or agents for most of the falls?
The evidence collected plays a very crucial role in ensuring that patient falls are prevented. The safety departments use this evidence to propose implementing patient safety practices that help prevent falls.
