Identifying a problem
The identified problem in the clinical environment upon which to build a proposal for change is hypertension in adults – age 18 years and above. Hypertension (HTN), also known as high blood pressure (HBP) is basically a persistent medical condition wherein the blood pressure within the arteries is raised. According to the Centers for Disease Control (2014), roughly 67 million persons or 1 in three American adults have HBP. Just about 47 percent of individuals with this medical condition have their condition under control. HBP raises one’s risk for stroke or getting heart disease; the 2 main causes of death for American adults. Hypertension is sometimes referred to as a silent killer given that it does not have symptoms or warning signs, and a lot of individuals do not know that they have the medical condition (WebMD, 2014). This is particularly imperative if one has a close relative with HBP. The risk factors for HTN are as follows: family history, excessive consumption of alcohol, obesity, as well as smoking. The common treatment for HTN includes Beta-blockers (WebMD, 2014).

(2) Pre-HTN: diastolic blood pressure is 80 mm Hg – 99 mm Hg; systolic blood pressure is 120 mm Hg –139 mm Hg. (3) Stage 1: diastolic blood pressure is 90 mm Hg – 99 mm Hg; systolic blood pressure is 140 mm Hg – 159 mm Hg. (4) Stage 2: diastolic blood p...
