Five Requirements within the HCA322_Anesthesia Consent Form
1 The first requirement within this consent form is a statement at the beginning of the form explaining the anesthesia process. The requirement is noted by describing the procedures that will be followed in the anesthesia process as well as a description of what is going to happen to the patient. In addition, the expectations that the patient should have during the treatment process is also included in this first requirement.
2 The second requirement is a description of the side effects or complications from the surgery involving anesthesia appearing in the second paragraph. It is noted by including detailed examples of any possible complications.
3 The third requirement appears after the description of the possible side effects specifically aspiration. This requirement outlines the measures that the client is expected to take in order to minimize the possibility of aspiration. The requirement is noted by including observation of the diet in terms of the duration when the patient should restrain from eating or drinking prior to the surgery.
4 The forth requirement comes just before the voluntariness part. This requirement is the description of the surgeon’s role in the surgery and other persons that would be reasonably involved in the process such as Anesthesiologists and Certified Registered Nurse Anesthetists (CRNAs). It is noted by defining the actions that the medical personnel would engage in.
5 The final requirement within this consent form is voluntariness appearing at the end of the consent form: This is requirement is gives the client an opportunity to make a personal decision without coercion.
