Mr. Shields had gone through three years of treatment for a rare form of lymphoma. Chemotherapy had not worked. Had not given up hope, but he was well aware of his diminishing odds. Doctors are more optimistic and seem to be concerned more with treatment than with discussing death. He was wary of having false hope, and he cautioned the doctors against giving him false hope. As the days went by and he understood the inevitability of his death, he did not seem afraid but clear on what he wanted to do, where he wanted to be, and who he wanted to be with. He is seen having a difficult conversation with his grandchild and giving him hope, which shows he was ready to face death and was a source of strength to his family and the people around him. He is the only patient featured in the documentary who could speak to the film's makers on his last day and still seemed he would be alive for a while longer.
Genie, Shields' wife. Shields wanted to make a decision. He did not want to leave it to doctors or fate. He once made it very clear to the doctor that he wanted to be on his farm and not in the hospital, even when his health was failing.
He had talked to his wife about his wishes and priorities. Genie hoped that she would fulfill his wish to be at the farm. Like Jeff, Genie had hoped but was also well aware of the diminishing choices they had.
Robert Soiffer was Shields' oncologist. He met him when Shields required a bone marrow transplant. However, his condition deteriorated, and his disease came back. Also, he had an additional issue where the donor cells were attacking his body, and he seemed to get frail by the day.
Shields made it clear that spending his last days in hospital was not appealing to him. He wanted to be with his family and on his farm.
Shields was quite thoughtful in his assessment and approach to his treatment and medication. He did not want to pursue therapy for the sake of pursuing it but wanted to pursue it if it guaranteed him a reasonable quality of life.
Genie also seemed well versant with the condition of her husband. As she was speaking, she says that they know the lymphoma is growing. She also worried that their being at the hospital could make it difficult for their children to have closure with their father.

While watching Jeff Shields' story, I noticed how the doctor, Robert Soiffer, was close to him and always allowed him to speak his mind. The autonomy principle mainly revolves around the idea or concept of independence or freedom in what the patient wants to do and the choices they wish to make without coercion.
