Nursing Care Plan for Chronic Illness and the Family Instructions: Develop a Nursing Care plan for the following patient scenario. As you prepare the plan of care, consider yourself a home health RN. Rather than focusing on a physiologic problem, develop a plan of care to address the impact of chronic illness on this family. Scenario: Evelyn Marshal is a 76-year-old female with a health history that includes cerebral vascular accident (CVA), chronic obstructive pulmonary disease (COPD), and osteoarthritis. She lives in an adult retirement community with her husband, George, who is 74-years-old; George is her primary caregiver. Her physical ability has declined and her symptom management is becoming more difficult to control. Previous treatments are no longer effective. You suspect that Mrs. Marshal is now in the downward phase of the Trajectory Model of Chronic Illness. George verbalizes he is worried about his wife's declining health. He wants to keep her as comfortable as he can but he is having trouble providing care. It prevents him from getting sufficient sleep. He is anxious, fearing he cannot meet her needs. Meanwhile, his own health is declining. Using the Overview of the Nursing Process and information from your textbooks, develop a Nursing Care Plan for the above patient. Your Nursing Care Plan (NCP) must include: Analysis: Identify and develop a priority three part actual nursing diagnostic statement, based on presented data that identifies health problems. Planning: Develop one appropriate measurable patient outcome that will reduce or resolve the identified nursing diagnosis. Implementation: Write four (4) nursing interventions, including one assessment action, one teaching action, and 2 actions to resolve the problem. Evaluation: Identify criteria that will be used to evaluate the effectiveness of the desired outcome and determine if the outcome has been met.
