  • Define reflection and the three phases of reflection. Cite the Asselin (2011) article in your definition.
  • In your own words, describe what reflection is.
  • Answer the following question: Why is reflection important to you as a professional baccalaureate-prepared nurse? Cite at least one professional resource from the learning resources to support your statement.
  • Support your statement about why reflection is important with an example. Describe one example from your own nursing practice where your nursing practice has improved because you reflected and grew from this experience.

Reflection – Descriptive Phase

  • Read Asselin (2011) to guide you in these three phases of reflection. The three phases of reflection are found on pages 5-6.
  • First, begin by describing in detail the clinical project issue/problem you are focusing on for your clinical project this semester.
    • The clinical problem defines the actual problem and does not include a solution statement.
    • Provide as much detail as needed to fully explain the extent and pattern of the clinical problem, but to maintain privacy, do not include any HIPAA or organization identifiers.
    • Description includes (a) What = health issue of concern, (b) Who = person/population having the issue, (c) Where = setting of the issue, (d) When = time, and (e) Why/how = potential causes.
    • When describing the problem, support your description by citing at least two credible, professional evidence-based references, in addition to online websites or institutional policies.
    • Next, reflect on current practice to provide a truthful account of the problem and possible contributing factors. This includes reflection on your own practice related to your clinical project issue and can also include experiences of others in your setting.
      • What are your thoughts and feelings related to this clinical issue? Why do you feel this way?
      • What are the thoughts and feelings of others? Consider key stakeholders involved in the problem. This can include, but is not limited to, other nurses, CNA’s, managers, leadership, educators, physicians, pharmacists, case managers, patients, &/or families.
      • What actions have you taken that could contribute to this issue and why?
      • What actions have others taken that could contribute to this issue? Continue to consider the key stakeholders and their actions.
      • Is there anything else you feel you should add to provide a thorough truthful summary that includes all contextual elements necessary to convey a full understanding of the issue?

Reflection – Reflective Phase

  • To receive full points on this section, you must address #1, 2 and 3 below. See rubric for exceeds and meets expectations criteria.
  1. Compare the standards surrounding the clinical project issue and your own personal beliefs, assumptions, and knowledge towards the standards.
  2. Examine the clinical project issue to identify how specific aspects of this situation influence practice.
  3. Reflective review of the intention or reasoning behind actions.
  • Answer the following prompting questions:

Reflection – Critical Phase

Best Practices

  • Search the professional literature for best practices surrounding the clinical project issue.
  • Identify at least 4 credible, professional, evidence-based references that enhance your understanding of best practices that can improve your clinical project issue. Online websites or institutional policies are unprofessional sources.
  • Using the literature, describe:
    • What is known about the topic?
    • Where are the gaps in knowledge?
    • What potential evidence-based solution(s) can be instituted to address the problem? Describe at least two evidence-based strategies that can be used to change practice and improve the problem.
    • Provide in-text citations as appropriate when you synthesize the literature findings.
    • Paraphrase the recommendations from the literature into your own words; avoid the use of direct quotes whenever possible. According to the American Psychological Association (APA) (2020), “it is best to paraphrase sources… rather than directly quoting them because paraphrasing allows you to fit the material to the context of your paper and writing style” (p. 270). Direct quotes should be reserved for instances “… when reproducing an exact definition…, when an author has said something memorably or succinctly, or when you want to respond to exact wording (e.g., something someone said)” (APA, 2020, p. 270).

Critical Reflection on Best Practices

  • Reflect on what is stated in the literature and answer the following questions:
    • Were your actions or the actions of others based on best evidence, the most appropriate and successful?
    • Do you or others use knowledge appropriately?
    • What factors got in the way of doing a better job and why?
    • Do you need to rethink your values and attitudes?
    • What have you learned from this situation?
    • How can you or others change practice in the future?
    • The goal of answering the above questions is to come to the following conclusions:
      • What do you know?
      • What are the gaps in your knowledge?
      • What are the practice problems?
      • What did you learn?
      • What additional knowledge do you need to improve similar practice issues in the future?
      • How will you act differently in future similar situations?

According to Asselin (2011), reflection denotes a nurse’s conscious thinking, assessing, and deriving useful insights from any clinical experience, allowing the nurse’s effective practice. She added that the nurse analyzes her opinions, attitudes, deeds, and feelings about a circumstance from the perspective of their experience, understanding, or assumptions. Reflection has obtained significant relevance in nursing education due to its capacity to promote learning. The reflective process entails exploring one's experiences, examining and discovering helpful insights to be skillful in the profession. Asselin’s three reflection phases are descriptive, reflective, and critical phases. In the descriptive phase, nurses describe their feelings, actions, thoughts, and main clinical scenario features. The reflective phase follows, analyzing reflection on the scenario and intentions and reflecting against standards. Lastly, the critical phase identifies the required nursing modifications, knowledge gaps, or means to incorporate new knowledge into future nursing practice.
For several reasons, reflection is crucial in professional nursing practice (Bulman & Schutz, 2013). For instance, as a qualified baccalaureate-prepared nurse, I can use the process to constantly enhance my nursing practice, hence long-term successful care for all patients. Reflecting on my decisions, experiences, and activities can allow me to recognize potential growth areas and develop as a nurse. I experienced a patient with problematic behavior and needs in one of my workplace shifts. Originally, I found it hard to communicate efficiently with the patient and offer the required attention. After my duty, I reflected on the situation by thinking about the scenario, analyzing my reactions, and assessing whatever I could derive from it. During this reflection, I acknowledged that I needed more effective communication skills to handle patient needs efficiently. Therefore, I generated an action plan that included finding appropriate mentorship from experienced professionals to identify relevant approaches to handling challenging
