Roger's diffusion of innovation theory is a particularly good theoretical framework to apply to an EBP project. However, students may also choose to use change models, such as Duck's change curve model or the transtheoretical model of behavioral change. Other conceptual models, such as a utilization model (Stetler's model) and EBP models (the Iowa model and ARCC model) can also be used as a framework for applying your evidence-based proposal in clinical practice. Apply one of the above models and carry your implementation through each of the stages, phases, or steps identified in the chosen model.
In 500-750 words (not including the title page and references page), discuss applying one of the change models to the implementation plan:
Identify the selected model or theoretical framework and discuss its relevance to your project.
Discuss each of the stages in the change model/framework.
Describe how you would apply each stage in your proposed implementation.
In addition, create a conceptual model of the project. Although you will not be submitting the conceptual model you design in Topic 4 with the narrative, you will include the conceptual model in the appendices for the final paper.
You are required to cite three to five sources to complete this assignment. Sources must be published within the last 5 years and appropriate for the assignment criteria and nursing content.
Prepare this assignment according to the guidelines found in the APA Style Guide. An abstract is not required.

Today, theorists have come up with different theories to understand some research studies. Some of these theories have worked while others have not. The paper describes the implementation of structured, intensive education maximizing engagement, motivation, and long term change towards reducing the cases of type 2 diabetes in the hospital based on Roger’s diffusion innovation theory. The phases include knowledge, persuasion, decision, implementation, and confirmation.
The diffusion theory by Rogers utilizes communication to come up with an explanation of how an idea gains motion and spread over the social systems with people adopting new ideas and approaches in how they do things. According to this theory, communication is very crucial when adopting the proposed ideas by achieving the desired objectives. This theory also explains how diffusion takes place through the population. Various characteristics are needed for the ultimate change to take place. These characteristics include observability, trialability, relative advantage, complexity, and comparability (Frost, 2017). Complexity is how change practices are learned and applied by the people. Observability is the positive results that are observed after the implementation of the proposal. On the other hand, trialability is a certain quantity of change that is used to determine the relative value of a proposal.

Type 2 Diabetes is a disease that keeps the body from using insulin the way it normally does. People who have type 2 diabetes are said to be insulin resistance. Young and middle-aged people are likely to get this disease. It affects young people because of childhood obesity. This is one of the most common type of diabetes in the United States (Shaw, 2019).
