Patient and Bio-Psycho-Socio-Spiritual Assessment

Essay Instructions:

I work at a substance abuse residential treatment facility as a counselor, and I am a social work major.
For this paper, I chose a client of mine named Michael. He came to my facility for the first time ever, trying to get help. He is 37 years old, he is an American Indian, and he suffered from abuse in his childhood and used multiple drugs since he was nine years old. His parents were both alcoholics and drug addicts. He also has many mental health diagnoses. I chose him because I have been in this field only six months, and he is the first client that I was able to see change in; I put lots of empathy, time, and compassion into his case.
Here are the main points in this paper; you can fill in everything else off of this.
