1 In what ways does he show signs of resistance to therapy?
DB, the patient, showed various forms of resistance to therapy. However, all of the signs and symptoms point to one central issue: denial due to fear. First, DB showed denial by the lack of acknowledgment of his poor emotional control and behavior. He always blamed the behavior of other people toward him, which resulted in his actions. This includes being annoyed with his girlfriend's daughter and their puppies because they were hard to deal with. Because he cannot confront that reality, DB opted to scare the girlfriend and her daughter by hurting the former. Eventually, DB burned the house and showed no remorse. He also frequently got into fights when he was an inmate, resulting in his frequent relocation to the facility. Furthermore, DB attributed the failed therapies to the insufficient knowledge and skill of the psychologist or psychiatrist. He thinks they are ineffective because they lack the experience or creativity to proceed with the treatment efficiently.
Second, DB masked his fear by thinking that he is above the others in all aspects. This is especially true when he was already an inmate. He thinks that all the other inmates were dumb or that they cannot read beyond the lines. He also thinks that he is ways smarter than his therapists. That is why the sessions never helped him because they are ineffective for him. This is all despite his IQ score of lower than average. His feelings of superiority aggravate his resistance to therapy because it means that no one can help him if they are less knowledgeable than him.
Third, he denies his fear of his father and denies that his behavior may be a result of physical and verbal abuse from him during childhood. DB iterates that the current therapist's notion is incorrect because he never thought it was "physical abuse." His denial turned to a lack of insight. This is despite DB telling the therapist in detail how his father would hurt him before.
All of these were contributory and showed a domino effect. Thus, resulting in resistance to therapy. Lack of acknowledgment of fear must be the primary objective to prevent resistance to therapy.
2 Did this resistance affect the direction of therapy?
The resistance effect negatively affected the direction of therapy. As previously stated, all the other therapists that he had reported a similar problem with DB. Fear demonstrates vulnerability, and DB's way of masking it is to show that he is dauntless in all possible ways, such as aggression, violence, and physical and verbal abuse. All of these show his "stronger" side. Hence, it masks his vulnerability. Because DB was always afraid, he was never open to options. His only option was to become strong because he will get hurt once he showed even a little sign of fear.
