Interview a volunteer subject and write up the Psychosocial History and MSE. You may use a friend or a family member. Please disguise all identifying information to protect the privacy of the person being interviewed.

For this assignment, download the following materials from additional resources: Interview Guide and MSE. The Interview Guide can be used to help guide your interview questions. The MSE form can be used to conduct the Mental Status Exam. The information you obtain from these forms will be integrated into writing the Psychosocial History.
Your subject should meet the criteria for at least one of the following disorders (you may need to fabricate some information):

1. Mood Disorder
2. Anxiety Disorder
3. Somatoform Disorder
4. Schizophrenic or Other Psychotic Disorders
5. Personality Disorder
6. Substance Related Disorder
7. Adjustment Disorder

Your report should be 4-5 pages in length. The report must include the psychosocial history, MSE, Diagnostic Impressions, Summary and Recommendations (testing section is not required). You should generally format your report according to APA style, but there is no need for references in a report of this type. The Sample Report that is provided under the additional resources tab is a guide. Your report may be more detailed in comparison to the sample report. Please be sure to provide sufficient information of your client’s symptoms to justify the diagnosis.

All assignments MUST be typed, double-spaced, in APA style, and must be written at graduate level English.

PSYCHOLOGICAL EVALUATION
IDENTIFICATION AND REFERRAL:
Jay Rodriguez is a 36-year old Caucasian man. He was driven to the facility for examination by his wife. He reported that he had previously received services from his personal psychiatrist and recommended for further examination so that he could receive a teaching credential. He recently moved to the United States for a teaching job and the school where he is attached requires him to obtain additional credential that is needed for coursework. As such, he has come for reassessment. He has history of mood disorder, depression, and mild anxiety. The current assessment is done to establish his current mental health and status and his capacity to remain stable and finish additional coursework. The school is also monitoring him for a potential promotion in the near future.
HISTORY:
Jay Rodriguez provided this history, although he was accompanied by his wife. He was stable enough to give it.
Family History
Rodriguez is married to one wife. The couple has lived together for 19 years and they have two children, aged 16 and 12. Rodriguez reported that he had a history of mood disorder, hot temper, and alcoholism. These issues became almost out of control in 2018 until the wife sought divorce or at least for Rodriguez to leave her together with the children. However, he is recovering.
Rodriguez was raised by both parents but his father passed on in 2014, and his mother also passed on 2 years later. Rodriguez was born in Mexico but his parents moved to Kentucky when he was 1 month old.
Educational and Vocational History
Rodriguez has a Bachelor’s degree in Art and Literature from the University of Kentucky. He also obtained his teaching credential three years ago. Before obtaining the teaching credential, he worked as a waiter in one of the top restaurants in Kentucky. Currently, he teaches English and literature to 7th and 8th graders, mainly in charge of students taking English as a second language. He also expressed his desired to further pursue his studies so that he can obtain a major in the field and continue teaching, and position himself for promotion. The major he intends to pursue is purely on teaching English as a Second Language, and so he will be dealing with international students.
Medical and Mental Health History
The medical history of the patient, as shown from the previous psychiatrist, shows that he had previously been diagnosed with high cholesterol, fibromyalgia, as well as a slight valve issue. As such, he had been put on medication (Vivactal), which is a tricyclic anti-depressant. He was also on other medication; Alprazolam and Buspar, which were to be taken when necessary. He had undergone surgery on his left arm due to an accident incurred when driving.
His mental health treatment history showed that he started taking psychotropic medications in 2013, administered by his general physician because he showed signs of mood disorder due to lack of self-control. He was also involved in heavy drinking in 2014, and this made his behavior out of control as he could not manage his temperaments.
