Need to submit a training evaluation form for CPI (Crisis Prevention Institution) training that helps us, as school employees, who work with children who are behavioral. In this training, we are taught the proper steps to take when we see that a child is going to have a breakdown. These break downs could stem from anything and as long as we are taught how to see the signs we can try and help the child deescalate. Along with this training, we are taught the correct way to transport a child who is at risk of hurting themselves or someone else and what needs to be done as follow-through. This training helps staff member's know how to handle various situations that they may end up in. This is a 2-day training and we have to be tested on what we learned to be sure that things are being done correctly in case we are ever in a situation where this training is needed.

This is a training evaluation form that is designed to appraise the effectiveness of two-day training at a Crisis Prevention Institution. The training was purposed at helping the employees of the school in dealing with students who have behavioral issues. The evaluation measures specifically the effectiveness of the training by looking at the level of information retention amongst the trained staff.
