The role of nursing practice is changing to patient-based. The aging population creates high demands for more and better health care services. This group of aging population faces chronic conditions such as arthritis, diabetes, hypertension, mental health conditions, and cardiovascular diseases creating a need for long-term and palliative health care. The affordable health care act of 2010 increased expectations for health care professionals and provided the opportunity to develop patient-based system. The transformation to patient-based systems requires; more primary care as opposed to specialty care, more community-based care, and enabling nurses to practice to the full extent of their training, competencies and education (Wood 2011).
