The health record is a form of communication between internal and external stakeholders in the healthcare environment. Health informatics professionals are responsible for managing health records whether in paper, electronic, or hybrid format. Guidelines must be followed in maintaining records, including their storage and destruction, and policies must be evaluated for their compliance with state and federal regulations.
Task:
A. Discuss how you would manage the use of paper forms in a hybrid environment in order to maintain the integrity of the health record.
1. Compare the strengths and weaknesses of using hybrid records.
2. Discuss legal issues that may arise when using hybrid records.
B. Evaluate the attached “Willow Bend Record Policy” to determine if it protects health information for record storage and destruction of paper and electronic health records based on the following:

In the hybrid environment, all data must be managed in a way that integrity of health records is maintained. To achieve this, patient information on paper records should be processed, coded and monitored appropriately. The records should be stored in such a way that they are accessible to the authorized persons whenever and wherever that information is required (CPSA, 2004).
There are strengths and weaknesses associated with the use of hybrid records. Medical records play crucial roles such as maintaining patient care’s history, supporting the work flow for the physicians and staff, supporting communication with the external sources such as radiology clinics and laboratories that provides additional medical information and also enables referrals.Having both paper and electronic records is advantageous in that if data in one form of storage gets lost, one can always retrieve it from the other form of storage.Managing health records in hybrid environment has its own challenges especially due to the transition management requirements. It is expensive because it requires duplication of records from paper to electronic information records. The process of compiling and retrieving information is laborious and prone to errors (CPSA, 2004).
Legal issues may arise when physicians are required to compile records within a short period. It is so cumbersome to get the pieces of information from both analog and electronic storage and a physician may be sued for leaving out some critical information in case of legal cases. In addition, some records may be destroyed because time dictates so whereas part of that information in different storage remains and this may raise a legal concern (Eramo, 2012).
