Continuum of care
First colleague
One of the major critics of the healthcare system today is that it seems to concentrate more on the episodic form of attention instead of incorporating the continuum of care. Continuum of care means that patients are followed up on the disease that is affecting the caregiver considers all the reasons for the illness and how to stop it especially chronic diseases. The caregiver should focus on the establishing a patient/provider relationship making the patients comfortable with the treatment program.
Second colleague
The healthcare system today does not offer follow-ups in patients with acute diseases they simply offer episodic care where they treat and discharge the patient. Following up on the patients is imperative towards preserving the lives of patients suffering from acute diseases (Mehta, 2012). This is because at times the cause of the illness is something minor for instance elevated stress levels which can be worked on through follow-ups. She argues that despite the changes in the nursing industry people are still forgetting the existence of the continuum of care and should exist to help save lives.
Third colleague
She advocates for the continuum of care with the reason that episodic care is disadvantageous to all parties involved. Providers are unable to view the client holistically that is they are not able to put the family history and to put the social-economic factors into consideration. Patients with chronic illnesses end up having complications and paying huge hospital bills (sometimes paid by the insurer) for something that could have been avoided through primary care.
All their points concurred with my research, and they repeated what I had been researching. They were all well-informed that the current system has ignored continuum of care and were all advocating for it to be implemented.
Accountable care organizations (ACO)
These are groups of doctors, nurses and other healthcare providers that come together to offer voluntary and highly coordinated medical care to patients (Springgate, 2011).
