Since Brown and Olshansky published their seminal work in 1997, challenges of advanced practice registered nurse-practitioner (APRN-NP) shift to practice have been a focus in scholarly literature. One of the significant difficulties relates to the transition in competency level APRN-NPs encounter once they leave their registered nurse (RN) positions to start practice as new APRN-NPs addressing mental health issues. In addition, there is a tremendous cognitive transition from a competent RN to a novice NP. This professional identify adjustment from "expert" RN to "inexperienced" NP could undermine self-confidence, stifle professional role progression, and adversely impact novice NPs' decisions to continue the clinical practice. This paper delineates the differences in nursing roles of RN and APRN-NP, potential ethical issues and related challenges, the role of PMHNP in health policy and public education, and educational and certification requirements for PMHNPs.
Differences in Nursing Roles of RN and APRN-NP
At the interpersonal or intrapersonal level, RNs are tasked with facilitating team-driven primary care interventions and collaborations between healthcare professionals. Their frontline roles include collaborating with providers to augment information-sharing capabilities via face-to-face interactions. RNs also coordinate care in the hospital community and collaborative transition programs at the interpersonal level. This role is inherently motivated by the need to foster care access for vulnerable families and persons, especially in mental health, chronic illness management, and maternal or child health services. At the organizational level, RNs collaborate with community stakeholders to foster care coordination, including working with other healthcare professionals to augment horizontal and vertical care continuity, especially in ameliorating social determinants of health for mentally challenged patients (Swanson et al., 2020).
On the other hand, the APRN-NP refers to an established medical provider tasked with assessing, diagnosing, ordering, and interpreting laboratory tests, as well as prescribing medications for respective patients in a partnership framework with other healthcare systems and providers. In addition, APRN-NPs also initiate and manage treatment, coordinate care, offer to counsel, and educate patients and their households on healthcare issues. They are licensed independent professionals who practice in coordination and autonomously with healthcare professionals as well as other individuals. APRN-NPs offer various healthcare services (e.g., diagnosing and managing complex, chronic, and acute health issues), illness prevention, as well as health promotion.
