Situation
The situation which I encountered was with a 90-year-old, female patient who presented at the hospital with left humerus fracture due to a fall at her home. Due to being in a hurry, there was one particular incident in which I forgot to put on my gown and gloves when attending to this patient. After providing her breakfast, I went for a 30-minute break and when I returned, I found the patient’s call light flushing. I immediately rushed to her room and found her standing between the chair and the bed, which was very dangerous because she could fall. She wanted help to move to her bed to sleep because she felt very tired. I then realized that I had not put on my gown and gloves. I quickly rushed back to the washroom, washed my hands thoroughly and came back with another nurse. We then assisted her to get to her bed and sleep. In general, I usually put on my gown and gloves every time I enter her room, but this situation was different since I did not do that because I rushed to her aid afraid that she may fall if I delay longer.
Background
The main issue with this incident is that I was in a rush to help her and prevent her from falling, but I totally forgot about the PPE before entering the room. The patient and I had a great relationship. As I worked with the patient, there was a previous nursing experience with a similar problem which helped me provide nursing care in this situation. In the previous situation, I attended to a 78-year-old male patient who had injured his right knee after falling on stairs and used a walker to move around. He needed assistance to move from bed to chair and vice versa and I always ensured that he needs to buzz the call bell and wait for a nurse to arrive to help him do any activity. I believe that working on the situation, the role of the nurse is to educate the patient regarding fall prevention, and do every effort possible to decrease the chances of falling in order to prevent the negative consequences from it.
Noticing/Collecting data
Initially, I noticed that this patient was trying to move herself without assistance which was a very dangerous move. It is worth mentioning that this patient had a high risk of falling. After spending more time with this elderly patient, I realized that when her condition improves and is discharged from this hospital, she would need to be taken to a long-term care facility considering that she does not have any family member to take care of her and assist her with activities of daily living.
