Purpose
The purpose of the iCARE Paper assignment is to explore the concept of interprofessional teams and patient outcomes. Nursing supportive actions of compassion, advocacy, resilience, and evidence-based practice will serve as a way to apply care concepts.

Interprofessional partnership plays a huge role in the safety of the patients. The patients would be at risk if there was no communication between people of different professions. Coming up with initiatives in health care centers can be a hectic process. Nurses, in the other hand, can find it hard dealing with the patients and medical practitioners. Applying Icare concepts can play a significant role in bringing a positive outcome in the service delivery of the patient. These concepts include compassion, advocacy, resilience and evidence based practice. Being a nurse can be a hectic task, but the above concepts help make it easier and promote effective service delivery. The healthcare sector is evolving fast. This means that the concept of healthcare providers from different disciplines getting together to form functional teams is also changing. Interprofessional collaboration involves working together and collaboration from people in different professions to deliver effective patient care. The essay discusses using Icare concepts to contribute to Interprofessional teamwork and patient outcomes.
In the clinical setting, I was working at; there was no Interprofessional team. I therefore decided to come up with ways to create a team that will include all the medical practitioners in the hospital. The aim of the team was to promote communication and to serve as a platform for issues affecting the patients and the hospital at large. The stakes in the healthcare sector are higher. This means that patient care delivery methods need to be improved to suit the needs of a patient (Hood et al., 2014). In the process of creating the team, I decided to include an interdisciplinary team will help guide the team members and prevent any incidences in the future. In cases where there will be issues, the interdisciplinary team will punish the member without bias, since the members will represent the different professions making up the team.
Compassion, advocacy, resilience, and evidence-based practice are some of the core aspects that will help me contribute to the team and the patient outcomes. All the health care providers are supposed to treat their patients with respect and most importantly, show compassion. There are nurses and healthcare providers who treat their patients badly, who end up creating a bad impression of the healthcare facility.As nursing professional, I found it important to show the patients that I care for them and I was available whenever they need me.
