2. Describe the amount of time needed to complete this project. Create a timeline. Make sure the timeline is general enough that it can be implemented at any date. 3. Describe the resources (human, fiscal, and other) or changes needed in the implementation of the solution. Consider the clinical tools or process changes that would need to take place. Provide a resource list. Although you will not be submitting the resource list in Topic 5 with the narrative, the resource list should be placed in the appendices for the final paper.
4. Describe the methods and instruments, such as a questionnaire, scale, or test to be used for monitoring the implementation of the proposed solution. Develop the instruments. Although you will not be submitting the individual instruments in Topic 5 with the narrative, the instruments should be placed in the appendices for the final paper.
5. Explain the process for delivering the (intervention) solution and indicate if any training will be needed.
6. Provide an outline of the data collection plan. Describe how data management will be maintained and by whom. Furthermore, provide an explanation of how the data analysis and interpretation process will be conducted. Develop the data collection tools that will be needed. Although you will not be submitting the data collection tools in Topic 5 with the narrative, the data collection tools should be placed in the appendices for the final paper.
7. Describe the strategies to deal with the management of any barriers, facilitators, and challenges.
8. Establish the feasibility of the implementation plan. Address the costs for personnel, consumable supplies, equipment (if not provided by the institute), computer related costs (librarian consultation, database access, etc.), and other costs (travel, presentation development). Make sure to provide a brief rationale for each. Develop a budget plan. Implementation is the process of putting proposed solutions into use with the aim of achieving the desired objective (Houser & Oman, 2011). The research uses the Academic Centre for Evidence-Based Practice star model in adopting the proposed solution for lowering the number of falls by the elderly in hospital environments. The proposed solution is a continuous assessment and reduction of fall hazards. The Ace Star Model involves five stages of implementation, which include discovery, summary, translation, integration and evaluation. The model is based on the transformation of knowledge through the various stages of solution implementation (Rycroft-Malone & Bucknall, 2010).

Knowledge Discovery: research is undertaken using both qualitative and quantitative techniques in order to obtain a better understanding of the situation at hand and the proposed solution. It is important to know the number of falls recorded over a particular period and their associated causes (Health Research & Educational Trust, 2016). The facts concerning the proposed change should also be understood before implementation starts.
Evidence summary: in this stage, the findings achieved from the qualitative and quantitative analysis done in the first stage are summarized accordingly (Rycroft-Malone & Bucknall, 2010). It is necessary that the number of falls over a given period and the causes are clearly presented so as to inform the best solution approach. The common causes of falls among the elderly are the most crucial information needed to determine the solution approach.
Translation: the information collected is combined with other available information that includes clinical expertise, and translated to suit the situation at hand (Rycroft-Malone & Bucknall, 2010). The primary causes of falls in the hospital by the elderly and their prevalence rates help in providing a safer environment for the admitted elderly population.
Integration: the proposed solution is implemented through both formal and informal techniques, putting into consideration the organizational and individual factors that influence the adoption of the proposed changes to the system. It is important that the proposed solution goes hand in hand with the hospital policies and operations, thus avoiding inconveniences (Titler, 2008). Since the solution involves assessment and reduction of hazards, the new roles are integrated into the day-to-day duties of the employees, thus enhancing success.
