The 65 year old patient was admitted and the problems listed were noted from 2016 to 2018 and this included acute congestive failure, which was unspecified, ESRD (end stage renal disease), essential hypertension and shortness of breath. In the first assessment the principal problem was identified as ESRD (end stage renal disease).
