The Care Model Process model is a replacement for the reactive, visit-centric approach known as the Wagner Chronic Care Model. The Care Model Process allows HealthPartners nurses to analyze electronic health records and also order lab work done before the patient’s appointment. The nurses are also responsible for coordinating the between-visit and post-visit care, as well as traditional care after hospitalization. Based on the model, patients can get a comprehensive office visit that lasts for several hours (Robert Wood Johnson Foundation., 2012). The nurses assess the current concerns before the patient meets with the primary care providers, and they can then follow up to make sure that patient is knowledgeable and knows how to implement the patient care plans.
