The purpose of the graded collaborative discussions is to engage faculty and students in an interactive dialogue to assist the student in organizing, integrating, applying, and critically appraising knowledge regarding the diagnosis and treatment of mental health disorders. Scholarly information obtained from credible sources and professional communication are required. Application of information to professional experiences promotes the analysis and use of principles, knowledge, and information learned and related to real-life professional situations. Meaningful dialogue among faculty and students fosters the development of a learning community as ideas, perspectives, and knowledge are shared.

Review the clinical scenario below and address the questions that follow.
Mark is a 37-year-old client with a 15-year history of schizophrenia. When he was first diagnosed, he was prescribed a second-generation antipsychotic medication. He complained about drowsiness and difficulty concentrating. He experienced a 40 lb. weight gain over a year. His girlfriend complained about his reduced sex drive and ended their relationship. He tried several different medications alone and in combination but found the side effects intolerable. He stopped taking his medications and started self-medicating with alcohol. Over time, he experienced several relapses and his condition declined markedly. He lost his housing and began living in his car. After being involuntarily committed following a psychotic episode, he was referred to the PMHNP at the outpatient clinic. When he presented for his appointment, he told the PMHNP that he stopped taking his medications due to their side effects.

Goals that are not mutually shared between providers and patients can be very challenging, both clinically and emotionally. What steps can the PMHNP take to promote their personal emotional and mental health when working with clients whose health choices are likely to lead to adverse health outcomes?

Effective psychiatric mental health nurse practitioner (PMHPN) response should consist of a biomedical and biological psychiatric view and a humanistic approach consisting of human psychology with an emphasis on best psychosocial approaches. Gonzalez-Pando & Alonso-Perez maintains that the biomedical and biological perspective fulfils the diagnostic criteria as a dichotomic decision. At the same time, the dimensional approach is the more traditional approach with psychosocial interventions embedded in recovery (2018). Therefore, in this case, the PMHPN should provide an integral and humanistic approach centred on Mark’s reactions and responses factoring in interpersonal therapeutic relationships. Cort et al. (2021) note that patients' concerns and attitudes towards medication should be explored with a clear rationale for the proposed medications. For example, managing such a patient with atypical antipsychotic medication in late schizophrenic-like psychosis involves examining extrapyramidal side effects, evaluating after two weeks of medications, continuously reviewing compliance, and reporting and discussing concerns with the psychiatric team. Lastly, non-pharmacological approaches should involve the engagement of social services staff, community mental health teams, and caregivers.
The PMHPN Adherence Support for the Client
First, PMHPN should collaborate with Mark and the medical team to establish an optimal, effective, and acceptable treatment and management plan. The patient has complained explicitly about medication side effects in this case study. Hence, support and informed adherence can be encouraged via open communication and routine patient education, including updated medication, routine, and structure (Cort et al.,2021). Cort et al. (2021) maintain that nurses and case managers with training on CBT can apply CBT.p to older adults with VSOSLP since increasing PMHPN repertoire skills in response to psychotic experiences can be beneficial in the continuance of care. Additionally, in ongoing support, nurses can develop stress vulnerability models to help patients make sense of psychosis and recognize triggers to reduce the likelihood of relapse. Generally, service pathways and discharge processes should be planned and discussed collaboratively with the patient, general practitioners, and relevant agencies like social support systems and caregivers.
