Evaluation
Patients will receive education on purchasing medications at a lower rate during their visit. During the intake stage of their appointment, all medications are reviewed per standard procedures. Physician offices will now implement and exit intake before the patient leaves. "There are five general steps that have been identified as a guideline for evaluators developing an evaluation plan. Those steps include to assess context, gather reconnaissance, engage stakeholders, describe the program, and focus the evaluation (Holden & Zimmerman, 2009)." The evaluation process which would be used to verify the effectiveness of the change would be continual follow-up. Patient surveys, intakes and exits would allow one to assess context. Medication resources such as pamphlets and or brand name generic awareness allow one to gather reconnaissance. Meetings and educational intakes would engage stakeholders.
Dissemination Plan
When sharing the outcomes with an organization it is important that the following things continually occur: Communication, follow up and analysis should be conducted weekly and bi-weekly updates with stakeholders should occur. Information, data must be gathered and given to stakeholders. When sharing my outcome, the results of educating patients on how to purchase more cost-effective medications thus lowering their health care costs, patients survey are going to be crucial. Patients will have the opportunity to fill out a survey and or ask any questions during the exit interview. This information will be gathered and provided to the stakeholders. Very important that there is consistent and continual communication and follow up (Balasubramanian, Cohen, Davis, Gunn, Dickinson, Miller, Crabtree, & Stange, 2015).
