Acute Care for Elders (ACE) is a model that incorporates interdisciplinary corporation. It also allows the caregivers to formulate other strategies that they can use to improve health care. Developed in 1990, ACE had four core principles as the cornerstone it was built on (Susan 2013). The care provided was centered on the patient; the team that could partake in the caring was interdisciplinary, discharge plans were part of the care, and the environment at the hospital was elderly friendly.
The patients who benefit from the ACE care are identified using eligibility criteria. This is done by the Emergency Department (Angela & Judith, 2006).
