The RN to BSN program at Grand Canyon University meets the requirements for clinical competencies as defined by CCNE and AACN using nontraditional experiences for practicing nurses. These experiences come in the form of direct and indirect care experiences in which licensed nursing students engage in learning within the context of their hospital organization, their specific care discipline, and their local communities.
As the country focuses on the restructuring of the U.S. health care delivery system, nurses will continue to play an important role. It is expected that more and more nursing jobs will become available out in the community, and fewer will be available in acute care hospitals.
Write an informal presentation (500-700 words) to educate nurses about how the practice of nursing is expected to grow and change. Include the concepts of continuity or continuum of care, accountable care organizations (ACO), medical homes, and nurse-managed health clinics.
Share your presentation with nurse colleagues on your unit or department and ask them to offer their impressions of the anticipated changes to health care delivery and the new role of nurses in hospital settings, communities, clinics, and medical homes.
In 800-1,000 words summarize the feedback shared by three nurse colleagues and discuss whether their impressions are consistent with what you have researched about health reform.
A minimum of three scholarly references are required for this assignment.

The nursing profession contributes around three million people to the nation’s workforce, and the figure is expected to rise gradually. Indeed the industry has a bright future as some recommendations have been made to ensure that nurses actively contribute to the health sector. The paper's primary objective of this assignment is to educate nurses on how the industry is expected to change and grow. It will additionally include views by some of the colleagues.
Informal Presentation
Concepts of continuity or continuum of care
The current healthcare system has been receiving criticism and is accused of providing more episodic care that is providing medical care to patients with acute illnesses without conducting follow-ups or bothering to provide a patient/provider relationship. However, with the introduction of some recommendations, this is expected to change. Nurses have been awarded more powers in that they can be full partners with other medical practitioners. This means that a nurse can stay in touch with a doctor treating a patient with a chronic illness and therefore be in a better position to do a follow (Nursing theory - remembering our future, 2014). The partnership between the two also makes communication easier; thus, follow-ups will be easy and quick as all the information is available from the doctor.
Accountable care organizations
In 2014, the department of health introduced a new bill that will ensure better medical care provision to patients through Accountable Care Organizations (ACO) ACOs are to provide a platform for teamwork and coordination in all medical settings including ambulatory services, long-term care, and acute care. This will ensure that patients suffering from chronic illnesses will be provided with long-term care, which means follow-ups and other services required by patients. They are also supposed to increase the quality of medical care while reducing the costs (Sutter-Barrett, Sutter-Dalrymple, & Dickman, 2015). The ACOs are ensuring a bright future for nursing as nurses will have more roles.
Medical homes
Critics are arguing that the current healthcare delivery system is not capable of providing sufficient workforce, reduce costs and curb the rapidly increasing cases of chronic illnesses. Those on the front lines are the ones facing most challenges particularly those in primary care. An example of a solution is the introduction patient-centered medical home model, which aims at providing coordinated and comprehensive care based on a collaborative relationship. This will ensure that nurses are given more roles in the homes to make sure that they interact with patients for better provision of long-term care (Hoof, 2014).
