Problem stream
In the Kingdon Model problem stream, policy problems that need attention are identified in the policy process. In California, the nurse practitioners function within the scope of practice following the protocols of the Nurse Practice Acts. The stakeholders understand that there is limited scope for all registered nurses as specified by the CA Nurse Practice Act. The stakeholders view the restrictions as a problem since the nurse practitioners do not maximize their skillet when there is narrow scope of practice. For instance, nurses are not allowed to admit patients to hospitals and this may restrict inpatient practice. One of the issues that highlight the problem of limited scope of practice among the nurse practitioners is that nurses lack of autonomy. However, dealing with the issue is difficult as there is still resistance among other health practitioners when collaborative practice is required. Still there is a need for more primary care providers who can work independently as there is shortage of nurses (Carthon et al., 2016). To address the issue there have been attempts to introduced legislation that increase nurse practitioner autonomy, while they have failed they have shown that there is a need for change.
Policy stream
The policy stream focuses on the potential solutions to the problem that originate from the experts, policy makers and the lobby groups. In 2013, Sen. Ed Hernandez (D-West Covina) proposed a bill to expand the role of nursing practitioners in California, to allow nurses more authority with less doctor supervision (Bartolone, 2013). While nurses require advanced training, eliminating the requirement to seek doctor supervision was opposed by the legislators and even the president of the California Medical Association (Bartolone, 2013).
