The US’s nursing profession is currently one of the largest segment of the health care workforce with close to 3 million subscribers. Despite the large number of members, the nursing profession experience challenges such as ineffective leadership, inadequate nursing practice, poor interpersonal collaboration, and increasing diversity that prevent nurses from responding effectively to rapid changes in health care settings (Pullen, 2017). Amidst these barriers, professional nurses continue to play vital roles in helping health care organizations realise the goals outlined in the “2010 Affordable Care Act.” However, there is still need to eliminate some of these barriers so that nurses can be well-positioned to lead the change initiatives and promote health care advancement.
The Work of the Robert Wood Johnson Foundation Committee Initiative (RWJF) and the IOM Research
The RWJF and IOM report was a two-year plan that was developed as a response to the challenges nurses were experiencing. The report focussed on the assessment and transformation of the nursing profession as well as the development of action-oriented teams that could help improve the future of nursing. Throughout the deliberations, RWJF and IOM committee established four fundamental issues that were structured in the form of recommendations.
First and foremost, the report identified the need for nurses to practice according to their levels of education and extent of training. Given that terms of licensing and practice regulations vary from state to state, decisions regarding the scope of practice regulations should be developed in a way that they pose varying effects on the performance of nurses (Pullen, 2017). When it comes to scope and freedom of practice, the report RWJF and IOM report indicated that state regulations and policies have effects on the activities of nurses. For instance, those states that allow nurse practitioners to attend to patients and provide medical prescriptions without the supervision of physicians improve efficiency and effectiveness of practicing nurses. It, therefore, follows that the roles performed by nurses in most states are not only determined by their levels of education and extent of training, but also by the state regulations under which such nurses work.
Secondly, the RWJF and IMO report demonstrated the need for nurses to attain higher levels of education and special training. According to the two committees, promoting seamless academic progression is one of the ways through which the federal government and state governments can ensure that nurses deliver safe and patient-centred health care services. The fact that the needs of patients have become more complicated means that nurses must satisfy all the requisite competencies so as to deliver the required high-quality care. Such traits and competencies must include “leadership,” “health policy,” “system improvement,” research,” “collaboration,” and “teamwork” (Pullen, 2017).
