Review The Joint Commission resource which provides some guidelines for creating spiritual assessment tools for evaluating the spiritual needs of patients. Using this resource and any other guidelines/examples that you can find, create your own tool for assessing the spiritual needs of patients. Create a survey to assess the subject's spiritual need during the interview. The spiritual needs assessment survey needs a minimum of five questions that can be answered during the interview. During the interview, document the interviewee's responses. Submit the transcript of the interview. The transcript should include the questions asked and the answers provided. Be sure record the responses during the interview by taking detailed notes. Omit specific names and other personal information from the interview. Part II: Analysis Write a 250-500 word analysis of your interview experience. Be sure to exclude specific names and other personal information from the interview. Instead provide demographics such as sex, age, ethnicity, and religion. Include the following in your response: 1.What went well? 2.What would you do differently in the future? 3.Were there any barriers or challenges that inhibited your ability to complete the assessment tool? How would you address these in the future or change your assessment to better address these challenges? 4.Describe the spiritual experience you had with your patient, family member, or friend using this tool. How does this tool allow you to better meet the needs of your patient? 5.Did you discover that illness and stress amplified the spiritual concern and needs of your interviewee? A: It depends on each situation, but mostly faith and hope give one the encouragement to deal with a situation. My spiritual beliefs are essential to coping with stress, with the most significant spiritual belief being promises to the faithful that he will not abandon them.

A: It depends on the situation, but faith in God, the social support from friends and families

A: As earlier stated religion offers hope and my faith in God is based on the understanding that he will never let one suffer than they can handle. I believe that spirituality is an important aspect to be considered in healthcare, but both parties should respect each other’s beliefs. In times of sickness, spirituality and religion offer hope, as one faces the challenges associated with the illness.
Q: To what extent does the religious community offer support?
A: Support comes in all manner of ways including social support, emotional support and at times financial support. A community of people sharing the same beliefs makes it easier to fellowship and seems support from likeminded individuals, especially women of my age group.

A: Listening to patients concern is okay with me, but arrangements if can be made to meet a priest then this would be nice.
Part II Analysis
Mrs. S is a 62 year old widowed Catholic woman, living alone at home, but with the youngest son living one mile away. The interview was okay since there was trust between the interviewee and I. the spiritual assessment provided a foundation for further dialogue on the role of spirituality on healthcare. The success of the interview was based on open dialogue with the questions being open-ended. Additionally, identifying the key elements that gave the interview in times of healing was necessary and this is to be undertaken, regardless of whether the person is spiritual or member of an organized religious group.
