Case Study 1
Ms. A. is an apparently healthy 26-year-old white woman. Since the beginning of the current golf season, Ms. A has noted increased shortness of breath and low levels of energy and enthusiasm. These symptoms seem worse during her menses. Today, while playing in a golf tournament at a high, mountainous course, she became light-headed and was taken by her golfing partner to the emergency clinic. The attending physician's notes indicated a temperature of 98 degrees F, an elevated heart rate and respiratory rate, and low blood pressure. Ms. A states, “Menorrhagia and dysmenorrheal have been a problem for 10-12 years, and I take 1,000 mg of aspirin every 3 to 4 hours for 6 days during menstruation.” During the summer months, while playing golf, she also takes aspirin to avoid “stiffness in my joints.”
Laboratory values are as follows:
Hemoglobin = 8 g/dl
Hematocrit = 32%
Erythrocyte count = 3.1 x 10/mm
RBC smear showed microcytic and hypochromic cells
Reticulocyte count = 1.5%
Other laboratory values were within normal limits.

She reported to have experienced fatigue and had low energy, especially when she resumed playing golf. Other notable symptom includes shortness of breath, dizziness, high body temperature and increased heart rate. These are common signs experienced by people suffering from anemia. Ms.A further states that she usually experiences some of these symptoms during her menstrual cycle, a situation that has lasted close to 10 to 12 years. The fact that Ms.A has been experiencing these symptoms for long explains why she lacks adequate iron in her blood. Lack of iron is a major cause of fatigue and feeling weak most of the time (Thompson, 2010). Ms. A is among many women at risk of developing iron deficiency anemia due to many factors. The red blood cells transport oxygen to other parts of the body, if these cells do not contain enough oxygen, one can experience short breath (Thompson, 2010). The blood needs to contain adequate red blood cells for the body to be supplied with adequate oxygen throughout (Thompson, 2010). Losing blood every month means having limited iron required by the red blood cells, hence limiting the oxygen being supplied. According to the lab tests, Ms.A Reticulocyte count is at 1.5 percent, while the normal reticulocyte count should be 1 percent (Mayo Clinic, 2014). This can probably be caused by the heavy bleeding, she reported to have experienced during her menstrual cycle every month.
