Sallie Mae Fisher is an octogenarian who has been diagnosed with hypertension, as well as cardiac-related problems. Even though, the patient has been hospitalized numerous times, and there is a need for constant monitoring, there is concern that there are safety problems that may interfere with medical interventions. Fisher hears a knock on the door and tells the nurses outside to enter even whoever they are, as she keeps her doors unlocked. As Fisher stands up to usher in the visitors, she steps on a rug placed on the floor. This is a safety concern, since the patient may trip over and get injured further complicating her heath condition. There are papers on the couch, and these shows that there is need for better safety measures. They are haphazardly placed on the couch, and it is not clear whether there is someone who regularly cleans the house where Fisher stays. Fisher is living alone, but her daughter comes to visit when she is free, and since she has problems with mobility, she is unlikely to reach out people around her who can offer help. Hence, patient education will be necessary to improve her well-being in light of challenges around her.
Substantiating evidence/ assessment data for the problems identified
The case for patient safety in hospital settings is relevant for nurses, as they seek to improve the wellbeing of patients. However, nurses may have less power to ensure patient safety especially when the patients are not monitored. Patients need to have independence while functioning well, with a social support system that ensures their safety necessary (Ellenbecker et al., 2008). In any case, patient education ensures that that there is patient compliance. At the moment, the patient is skeptical about taking more medications. Patient education extends to patient safety, given that for the home care patient, the patient has more autonomy to make decisions on how interventions are implemented (Ellenbecker et al, 2008).
