He has difficulty maintaining diet restrictions and managing his polypharmacy. He has 4+ pitting edema, moist crackles throughout lung fields, and labored breathing. He has no family other than his wife, who verbalizes sadness over his declining health and over her inability to get out of the house. She is overwhelmed with the stack of medical bills, as Mr. P always took care of the financial issues. Mr. P is despondent and asks why God has not taken him.

CHF(Congestive Heart Failure) is a collective term that is usually given to the various types of problems that affect the muscle tissues of the heart. The disease is caused by a buildup in various conditions within the body. This paper is aimed at providing enough justification as to why Mr. P. is a patient with such a problem.
Approach to care.
CHF patients often tend to experience a lot of fatigue as a result of lesser heart activity and little supply of oxygen to organs.Care for such patients entails a regular and constant review of their conditions, to check on symptoms as well as the effectiveness of the medication. Psychological counseling can be another useful treatment in this case (Tuckett, 2013). Medication comes in to help strengthen the heart muscles, which are generally weak as a result of the condition. To ensure the right nutritional therapy, the patient needs to stick to the dietary recommendations of the physician. The diet should contain the necessary elements for the recovery and regeneration of cells. The most preferable types of food for the patient include grains, fruits, and vegetables. Herbs and spices are recommended instead of using salt. Once the patient gets a little energy, he may do some little exercises to restore the strength of the muscles.
Treatment plan.
A treatment plan is important when it comes to helping the patient ease the pain. Medication together with the nutritional therapy approach would be very appropriate for this patient since his condition is worse considering the fact that he is very old, and his disease is at a critical stage.
