In 500-750 words (not including the title page and reference page), apply a change model to the implementation plan. Include the following:
1. Rogers' diffusion of innovation theory is a particularly good theoretical framework to apply to an EBP project. However, students may also choose to use change models, such as Duck's change curve model or the transtheoretical model of behavioral change. Other conceptual models presented such as a utilization model (Stetler's model) and EBP models (the Iowa model and ARCC model) can also be used as a framework for applying your evidence-based intervention in clinical practice.
2. Apply one of the above models and carry your implementation through each of the stages, phases, or steps identified in the chosen model.
3. In addition, create a conceptual model of the project. Although you will not be submitting the conceptual model you design in Topic 5 with the narrative, the conceptual model should be placed in the appendices for the final paper.

Section E of this proposal introduces and describes the Stetler Model as a framework for the proposed evidence-based intervention. The model has five progressive phases crucial in implementing a change in any clinical setting (Stetler, 1994). Each phase will be discussed in detail and indicate how the nursing department in UnitedHealth Group can implement the Stetler Model to address the problem of falls among the elderly.
Phase: Preparation
Before application of Stetler Model, it is essential to affirm the need and priority for a change. Falls among the older patients in hospital environments and the treatment of injuries associated with such falls is costly (Burns et al., 2016). Moreover, falls among the elderly have resulted in deaths, which necessitates the introduction of an intervention plan. Research studies have established a positive correlation between elderly falls and injuries as well as deaths in hospitals (Cameron & Kurrle, 2007). The context for the implementation of research could be easily integrated into the present guideline intended for use by the UnitedHealth Group. The search for relevant evidence has been already completed, and there is statistical support that falls among the elderly are strongly linked to serious injuries or death.
Phase II: Validation
Some of the qualifying assessments have previously been documented in this paper. To validate this section, enough research sources have been examined to include quantitative and qualitative methodologies in an attempt to triangulate the data (Gawlinski & Rutledge, 2008).
