Patient falls are crucial problems that need to be addressed to avoid further injuries on the patients and even death in older individuals with chronic conditions. Patients at a higher risk of falling are those from surgeries, introduction to unfamiliar environments, placement on catheters, those with acute illness, and those under medication. The concept of patient fall should be looked into by identifying the necessary prevention measure (Castlen et al., 2017). After identifying suitable fall prevention practices, the project members can then proceed to the implementation phase. The ‘suitable practices’ refers to expert opinions on the best ways to prevent falls in the hospital setting. The fall prevention practices should meet the universal fall precautions, address the identified risk factors of falls among patients, and include a review of post-fall procedures (Castlen et al., 2017). The program can be implemented if it is compatible with the hospice’s priorities and desires. The hospices’ main focus is patients with acute conditions and whose primary purpose of being in the setting is to receive treatment and cure for their illnesses.
The team members should also understand that the goal of client safety practice; the fall prevention, aims at preventing additional injury and harm to hospitalized patients. They should, therefore, ensure that top-notch hygiene is maintained to curb the spread of nosocomial diseases. They should also ensure that the patients are familiar with the new environment and demonstrate the call light use.
