Assessment data: Patient is alert and oriented x3. I assess patient's physical condition or cognitive status, whenever a fall happens; History of falls; Mental status changes.

During my clinical Rotation on February 9, 2018 at ER Department. I have 73 yrs old female, she came to ER with EMS personnel. She was fell on the ground and call the emergency bottom that she is wearing all the time. She has a history of COPD, Hip replacement surgery and hepatitis C. During the assessment at the triage she stated that she was fell because she was weak. I establish IV and draw blood and get urine sample as the doctor order. Lab result revealed she has UTI and low potassium level. So. we figure out that she was weak bec of low potassium and has an infection UTI. I give her rosephin medication IV push and give it for 5 minutes window and 40 MEq of potassium as the doctor order. Then, doctor discharge her together with her medication Bactrim DS 800mg tablet 1 tablet orally 2 times daily by mouth. Then I give her fall prevention and Home Safety teaching such as clear walkway, install night light, use not skid mats, keep floor dry and clean, remove throw rugs and etc.

I also had to educate her and her caregiver in order to ensure that they knew the drugs to take and their importance along with other practices. I created a safe environment by listening and addressing to all the patient needs.
