He has been smoking "pot" and says he drinks to help him too. He tells you he is afraid that he will not get into Heaven if he continues in this lifestyle.

Family history: is very positive for diabetes, hypertension, and alcoholism.

This case is about a young patient, 23 years old, who displays a number of things. Not only is he a smoker, but also an alcoholic, a habit that he says, helps him deal with his anxiety. He is quite full of energy from the outside, even though his Body Mass Index (BMI) is above the average measure. He believes that he is not spiritually okay, and doubts whether he would see heaven if he went on with the lifestyle he was practicing. All these just added up to his anxiety. Based on the information provided, the patient doesn’t seem to possess any other weaknesses, apart from his predisposition to diabetes as well as hypertension.
Under any nurse-patient interaction, communication is the backbone. It sets the tone of how care will be given to the patient, and how the nurse will also know and keep track of the entire session. Unlike the general social interaction that can make up a conversation, communicating with a patient for the purposes of coming up with his history is an entirely different activity altogether. It comes with the goals of building trust and understanding, since the patient needs assurance that the information he shares is protected in an unwritten oath of secrecy and privacy. Diction and intonation are therefore, two important elements in the effort to strike a rapport with the patient.
As for the questions to construct the history, they need to be clear and objective. They should be designed in a way that offers the patient room to divulge more information. The following are questions that can be posed to the patient:
