He quit smoking cold turkey when he was 78 years old.
He is in your office for a general health evaluation. He reports ongoing challenges with ‘belching’ but other than that he conveys that he is feeling pretty good.
He is on no routine medications.
During your interview with the patient you note that he utilizes pursed lip breathing. At times you note a faint ‘whistling’ sound associated with his respiratory effort.
In conducting your review of systems he reports a cough, particularly in the mornings, productive for thick clear to white sputum.
When queried about shortness of breath he does indicate that he gets SOB more easily than he used to.
His breath sounds are course and diminished in the lower lobes bilaterally.

Subjective: 
Patient F complains of a 3-day history of rhinorrhea, sore throat, post-nasal drip, nasal stuffiness, and fatigue. She also reports a low-grade fever. In addition, f has a history of migraine headache and use ibuprofen to manage this problem. Finally, she has a history of MAXALT use, but she no longer uses it.
Objective: 
F is currently using the hormonal contraception approach. Her vital signs include 112/66 mmHg: pulse 68: respiration rate 18: temperature 98.7:SPO2 99% on RA. On examination, there is a clear nasal discharge, erythematous posterior pharynx, pearly gray tympanic membranes, and no tonsillar enlargement. Her breath sounds are clear and bilateral.
