I have completed the other sections for the paper. Below is the section that I need help with.

Community Diagnosis A diagnosis will be formulated which clearly states the problem. Be sure to use a format which states the related factors (“related to”) and the defining characteristics (“as evidenced by”). While the diagnosis does not have to be a NANDA diagnosis, many will have elements of this. Examples might include “Risk for imbalanced nutrition related to a lack of available fresh fruits and vegetables as evidenced by an 8 mile distance to the closest grocery store in the Rochester inner city community.” This was a community garden project. Use the follwoing template for your diagnosis:
Community Health Diagnosis (CHD) Formulate a minimum of one CHD with the community. Document the CHD using the recommended format in the text. Risk of __________ (A specific health risk or problem faced by the community). Among ________________ (Identifies specific community). Related to ____________________ (Characteristics of the community and its environment identified in the analysis of data).
Plan Document a community focused plan involving members of the community as partner. Organize a group made up of knowledgeable community members to identify and formulate goals and interventions for a community-focused health program activity
Community Health Goal(s)Identify a  measurable goal including specific objectives with outcome criteria. As with all nursing care plans, it is better to keep your goal manageable so they can be measured in the evaluation phase. Goals like “decrease high blood pressure in the community” are not as measurable as “develop a DASH diet nutrition education program to be implemented through the community outreach department at XYZ hospital starting in September.”
   
    Interventions Identify the intervention activities and resources needed to achieve the objectives and accomplish the goal(s).  Document a sequence of actions and a time  
     schedule for achieving goal(s). Interventions must be specific and indicate:
Who will implement the intervention?
What, how, and when it will be implemented?
How will people be notified of this? 
(Consideration should be given to education space needed, educational materials needing to be developed, advertising, and activity promotion through TV or radio)
Describe the proposed implementation of a project or activity.  Include teaching, development of educational materials, etc.
