I. Overall Program Description Provide a paragraph that summarizes your program. Be sure to include:

• The program's name

• Setting of the program (e.g., a clinic, a school, a factory, a community center)

• A brief description of the nature of the program and its purpose

Example: Project Confidence is an interactive program for adolescent men (aged 14-17) in Lake County who have been referred by their primary care provider after receiving a diagnosis of a sexually transmissible infection (STI). The participants will be engaged in a month-long program that takes place on four Saturdays at the Lake County Boys and Girls Club. During the program, these young men will be exposed to a range of program activities designed to increase the likelihood that they will consistently and correctly use condoms during sexual events and that they will pursue routine screenings for both HIV and other STI.

II. Participants A. Describe your participants in 1-2 sentences. Example: The participants in this program will be adult women who reside in Lake County and who have received a diagnosis of Type II diabetes within the 30 days prior to the start of the program. B. Present ONE measurable objective to describe your participants.

Examples: To provide the program to 50 women who, within 30 days prior to the start of the program, have received a diagnosis of Type II diabetes.

III. Participant Recruitment

A. Describe how you will recruit your participants in 1-2 sentences.

Example: Partners in this program include 25 primary care clinics in Lake County. Each of those clinics has a designated nurse who will make a referral to the program upon a woman's diagnosis of Type II diabetes.

IV. Program Components

Program components are the actual activities that participants will be exposed to during the program. In this week's lecture, these were discussed in more detail.

A. Describe the four specific activities (one for each determinant) that you have chosen for your program. Cite specific evidence from the literature to substantiate your activities. Be sure to describe the frequency of each activity (e.g., it will occur weekly for one hour, etc.)

B. For each component, provide a description of the specific determinants that are targeted and describe the evidence or other rationale to substantiate that this component is realistic for changing the determinant(s) it targets.

V. Program Inputs

Describe FIVE specific program inputs that will be required in order for your program to be implemented. List each input separately and provide a brief (2-3 sentence) description of each input and why it relates to specific program components.

VI. References

The program's name is "Cuyahoga County Cardiovascular Health Promotion and Disease Prevention." The interactive program will feature a month-long education campaign, which will be conducted in Metro-Health Center for Community health. Participants will include middle-aged adults and seniors diagnosed with any cardiovascular disease or who demonstrate risk factors linked to cardiovascular disease.
