Relative to patient assessment and management of Mr. P’s cardiomyopathy and congestive heart failure condition, there is need to develop the best approach that can help him with his situation even at home. The essence of cardiomyopathy and congestive heart failure, management lies in the ability of the nurses to use an approach that is based on serial assessment as well as responding to changes in the status of the patient. This also means that the approach has to be delivered on a multi-disciplinary platform. The team has to remain alert at all times to make sure that they arrest all the factors that can exacerbate the cardiac conditions. These are factors such the dietary provisions, polypharmacy, pitting edema and the moist crackles in the lungs that can easily worsen the situation (Yancy et al., 2013). The idea behind the multidisciplinary approach is that, there is need for different specialists to take care of the patient.
