Cardiac Tele-monitoring System Executive Summary
Congestive Heart Failure (CHF) is a chronic progressive condition that affects the pumping capability of the heart muscles. While the condition is referred to simply as “heart failure,” CHF specifically denotes the stage at which fluid assembles around the heart and makes the pumnping capability ineffective (Antonicelli, et al., 2008). As the population gets older, the number of patients that have been diagnosed with congestive heart failure is expected to rise at a fast rate. The incidence and prevalence of congestive heart failure has gone up significantly in the last one decade ths increasing the likelihood of the patients developing other heart conditions. While CHF is not life threatening on its own, the presence of other heart conditions can aggravate the problem thus creating a scenario where CHF patients require regular hospital readmissions.
Purpose of the Program
The realization that the number of hospital admissions, length of stay and the health associated costs for individuals diagnosed with cardiac heart failure is not sustainable, Sinai Community Hospital has adopted a leading role in the evaluation and management of cardiac heart failure. An executive summary that proposes the implementation of an outpatient cardiac telemonitoring system to efficiently deal with this group of patients is presented. A discussion of ways to better how the process could enhance the health care system in relation to cost, quality, access and value is presented. If effected, the program will help in reversing the trends that are associated with cardiac heart failure and will also help the Sinai Community Hospital to save on costs in relation to taking care of this group of people.
Requirements
At the national level, heart disease is one of the most probable causes of hospitalization among people over the age of 65 years and over 55% of adults that have been diagnosed with heart disease die within ten years after the symptoms first appear. Among adults at an advanced age, CHF causes recurrent hospitalization and prompts placement in controlled living settings. The transfer from hospital to home is an important time for intervention to stabilize patients and to aid them in creating disease self-management strategies. Programs that connect CHF self-management education with out of hospital tele-health monitoring have grown to become highly effective at achieving this objective and at lowering the level of hospital readmissions as a result (Jovicic, Holroyd-Leduc, & Straus, 2006).
Target population
The target population for care received from the proposed clinic would be patients that have been treated and discharged for cardiac heart failure at SCH. Once discharged, these people will go back home and will be at increased threat of readmission. In the first year of operation, an approximate of 40 people will be served. Based on the prevailing SCH CHF statistics, it is estimated that participants into the program will be of different ethnicities and different languages (Antonicelli, et al, 2008).
