Depression is a common psychiatric disorder in children, adolescents, adults, and the elderly. Primary care providers, not mental health professionals, treat the majority of patients with symptoms of depression. Persons who are depressed have feelings of sadness, loneliness, irritability, worthlessness, hopelessness, agitation, and guilt that may be accompanied by an array of physical symptoms as well as substance abuse. Identifying patients with depression and substance abuse can be difficult in busy primary care settings where time is limited, but certain depression/substance abuse screening tools may help diagnose the disorder.

Discuss a screening tool that can be used in the primary care setting that can help with the identification of patients with depression and or substance abuse. What is your responsibility as a primary care provider to this patient once a disorder has been identified?

A large number of screening tools are used in primary care settings to identify people with depression and substance abuse. However, the most common and best tool is the Patient Health Questionnaire, PHQ-9 (Spiegel, 2017). This nine-question document is used to collect data about patients’ physical and mental conditions and to determine whether or not they are bothered by certain problems (Steinberg & Rosenberg, 2017). No doubt, it is a safe, effective and good way to determine the seriousness of the problem. With the first two questions, we can eliminate depression and help people understand that substance abuse can never benefit them. However, it is important for patients to give positive responses to these two questions before we could move on with the next seven questions. These are simple screening questions, whose answers also need to be positive. Once a total of nine positive responses have been received, it becomes easy for us to diagnose the illness in a better way and plan care.
