The goal of the policy is to make sure that the issue of polypharmacy is eradicated. Due to the fact most of the elderly patients tend to have more than one condition that they are dealing with, they have several types of medication that they have to take (Patricia & Tricia, 2015). This calls for constant visits to the pharmacy which in some of the cases means multiple of them. As such, there is a risk of polypharmacy as there is little to no communication between the pharmacies (Policy Design Lab, 2018).
To increases the chances of policy being a success, there are a number of tools that are going to be quite handy. One of the tool is the use of technology.
