1 cardiac symptoms Male, 30 years old. Patient complains of sharp pain in the chest and difficulty in breathing when lying down. Reports pain with deep breaths, coughing or swallowing. Pain is relieved by sitting down or leaning forward. Experiences shortness of breath and pain in the back, neck or left shoulder. Difficulty in breathing complicated by cough. Parent says cough is a week old, and audible wheezing started 2 days ago. Wheezing only in expiration but patient has trouble speaking. Parent reports wheezing even when patient is sleeping. Patient has <1/week night time awakening, and symptoms occur throughout the day. Patient unable to play and has to stay in bed. Dog in household and father is a smoker. No history of asthma from mother, but father`s history is unavailable.
