OBJECTIVE 1: Utilize a diverse range of communication approaches including technology to facilitate effective communication with individuals, families, groups and with a particular emphasis on communities.


 



I have utilized a diverse range of communication by creating a PowerPoint about osteoporosis. I first created a Pretest about osteoporosis so that my audience could test their knowledge. This pretest would be printed and handed out to the audience.  After they have finished the PowerPoint, they would then go back to see what question they got right. I also created a handout with my notes, important facts, resources, and all the information they would need about the condition. The PowerPoint included information about the definition, causes, risk factors, prevention, diagnosis, and treatment. Once the PowerPoint was created, I uploaded the PowerPoint on YouTube so that anyone that wanted that information would be able to access it worldwide. This is also a good because they would always have access to this information even if they lost the physical copy. YouTube seems to be one of the best ways to get this information out because it reaches so many individuals, families, and communities. Putting this information out on social media and the internet is the future. But I also believe we should still have physical copies available to the community for people who are not as tech savvy.

OBJECTIVE 3: Apply leadership concepts, skills, and decision-making in the provisions of high quality safe nursing care and emergency preparedness


 



I have applied leadership concepts by developing a PowerPoint on HIV. I developed this PowerPoint because Florida and speicallifally my county has been having increased cases of HIV over the last 5 years. After looking through survey’s I have found that one of the most common reasons why HIV has been increasing is due to sharing needles, and not using protection during sex. Often patients don’t even realize that they might have HIV because initially the symptoms are common flu symptoms such as fever, chills, rash, muscle aches, sore and sore throat. The best way the community decided to get the word out about the increases numbers of HIV was through social media, the newspaper and educating children in the school systems. The community is also being proactive by providing needles and condoms at certain locations in schools, health departments, and at homeless shelters. When someone does test positive, we hand them a brochures, resources, and education material. One of the most common complaints that we get is that they can not get the care needed due to insurance. I have also created a pamphlet where they would be able to receive the care needed. Most importantly patients need to know how to prevent them from spreading their illness. The best way to prevent is abstinence, limiting your number of sexual partners, never sharing needles, using condoms, and taking pre exposure prophylaxis, and post exposure prophylaxis medications. To help prevent the spread we educate our nurses every 2 years on how to recognize the systems, the proper way to educate patients on their condition, and to make sure. When cases are diagnosed in the hospital doctors and nurses are required to report cases to the health department to record the number of new cases in certain areas. This helps us to make sure what areas are the most effected and what is and what is not working implementation wise.

OBJECTIVE 4: Participate in the development and implementation of imaginative and creative strategies to enable systems to change in a community setting


 



I worked with my peers on developing and implementing creative strategies on the high prevalence of sexually transmitted infections in adolescents’ boys and girls between the ages of 10-19. The problems that we have identified as reasons why there is a high prevalence of STIs in adolescents include poor health knowledge, and poor resources for protection. How we decided as a group how we are going to combat this problem is by making HPV vaccination available for people from the ages of 11-26 years old, screenings in school, expedited partner therapy to prevent reinfection after treatment, and sex education. We want to implement these interventions because complications such as infertility, pelvic inflammatory disease, and chronic pelvic pain can occur. The main goal is to prevent the worsening of the condition and to prevent reinfection. Expedited partner therapy is a program designed to treat the patient and their sexual partner once diagnosed with and STI by providing prescriptions or medications to the patient to take to his/her partner without the healthcare providers first examining the partner. We decided that it was imperative to incorporate sex education due to lack of knowledge of how to use protection, and disease. Surveys showed at a large portion of teenagers have never had the sex talk with parents on subjects such as diseases, protection, or birth control. We have identified one of the best ways to make protection accessible for this group is by having protection in locker rooms, and bathrooms. Protection would be offered to students free of charge. We also felt that HPV would be available to students at the nurse’s office at the school and health department with parental consistent.

OBJECTIVE 5: Collaborate effectively with peers, persons in the community, and members of the interdisciplinary health team


 



Throughout the semester me and peers have been communicating through what up app, group texting and discussion groups to complete our community health project. All of these forms of this communication have worked out really well for us because we are notified immediately of a message at all hours. If anyone has a question about their section, it is quickly answered by one of the peers. As a group we created a PAR stating the population at risk are adolescents’ boys and girls ages 10-19. There is a high prevalence of sexually transmitted infection in adolescents due to the lack of education, knowledge and resources. Our interventions included HPV vaccinations from age 11-26 ages of age, screening for STI`s while they are attending school, students participating in expedited partner therapy, and taking sex education while in school. Everyone contributed articles for each section. Once our group had finished the par. Our instructor approved of our topic. Once approved we divided the topics up evenly. We decided to complete our sections early so that each member of the group could read over our projects making any punctuation and grammar errors. Once the paper was completed, we put the paper through Grammarly to check for errors and plagiarism. We also having the writing center look over the paper. We were able to do a group video chat together as a group so we could all be involved in the final corrections. Our hope is that our project will spread the word on the high prevalence on STIs to decrease the incidence in communities nationwide.

OBJECTIVE 6: Demonstrate accountability, ethical and legal decision-making surrounding health care dilemmas


 



While I was completing my HIV PowerPoint, I had no idea that there was so much accountability, ethical and legal decision surrounding the HIV issue. When I started looking up information for my PowerPoint, I realized that over the years there has been several lawsuits.  All 50 states require health care providers to report new cases of acquired immunodeficiency syndrome to their state health departments. As of 1989 states also required reporting of persons infected with human immunodeficiency virus. Physicians that care for the patient and the laboratory that tested the patient’s blood for HIV antibody are responsible for reporting the information to the right people. This is done so that public health officials can monitor the HIV epidemic in your city and state.  18 states require the patient’s personal information such as name, and address. Most only required demographics. Many states have a partner notification which is where if a patient test positive is obligated to notify your sex and needle sharing partner’s. If partners are not notified the patient can be charged with a crime. For those reasons HIV/AID patients have submitted lawsuits because they felt like it was violation of their HIPPA rights. They felt like their condition was disrupted publicly and needlessly. It was ruled that reporting to public health, and sharing information for the purpose of treatment, payment, and exposure is all federally permitted. It was ruled because this is a highly contagious disease that doesn’t just affect your life but others that you have contact with. This is to help contain the virus and to notify people if they have been exposed.

OBJECTIVE 8: Uphold and justify ethical principles when using patient care technologies in a community setting.


 



While I was gathering information for my HIV presentation, I realized that over the years there has been several lawsuits. Due to providers and labs having to report HIV patients. Some states even report the names and address of the individual. I believe this to be ethical so that the public health officials can monitor the HIV epidemic in your city in the state. This shows the state government if they are having increases and decreases in new cases. We can use this data evaluate our intervention on what we are doing right and what we are doing wrong. The evaluation process should be done every year so we can evaluate if the new interventions are effective. I will also uphold and justify the ethical principle of gathering the information of people that patient had sex with or shared needles to be notified if the person that was being tested became positive. Its important to noted that these people are only notified if the patient test positive. Everyone has the right to know if they have potential been exposed to someone that has HIV so that they can be tested immediately. This helps keeps the number of new cases from increasing. Potential HIV patients at this point would be able to use the proper precautions to keep from spreading the virus. Proper precautions such as absences, use condoms, and do not share needles. According to the CDC HIV can stay undeatced for 10 years. People who are exposed should contact their doctor immeadiely This is important for patients to let their doctors know especially if the patient is a woman and is about to give birth or is about to have surgery. Patients should be notified if someone knowingly had HIV and gave you the illness they could legally sue.

OBJECTIVE 9: Formulate a personal life- long learning plan for professional nursing practice. Appraise the impact on communities through investigation of health care policies, including financial and regulatory.


 



One of the main reasons why I decided to do one of my PowerPoints on diabetes is due to sister in-law. She is a low income woman who literally has to work to one job to pay just for her insulin and pump. Half of her income goes into paying for her insulin. Insulin prices have skyrocketed. 15 years ago a patient with diabetes might have paid 175 for a 20 ML vial of long acting insulin. Today the same tiny vial is now 1500 dollars. In 2016 the average price per month for insulin was 450. Patients have reported that due to the high cost they have are having to skip doses to be able to afford medicines. As nurses one of our main priorities is being an advocate for our patients. Nurses for years have been voted the most trusted profession. Due to nurses having knowledge, hand on experience, and involvement in the community. This makes them the prime candidate to advocate for their patients. One way that we can do this for our patient is by joining the American Nurses Association Political Action committee this committee support federal candidates who share our concern about the community. This gives us a voice in congress. Nurses should become an advocate at the local, state, and federal level. Ways that we can do this is by making phone calls to elected representatives about bills and testifying before committees. It would also be a good idea to become involved in councils or boards in the community, and workplace.  The nurse should be well informed about the statistics, and studies on the subject. Always have a plan on how to correct the situation. One main example of this Colorado who nurses fought to have to set a limit on the price of insulin at a 100 dollars. Be knowledgeable in all policy involving your patients so you can help them find the right care.
